We accept most insurance policies. Co-payments are collected at the time of service.
If you are unable to keep a scheduled appointment, please call 24 hours prior to your appointment time to notify us of your cancellation so that another patient may be seen instead.
If you are a new patient or have not been seen at one of our offices within a 12- month period please arrive 15 minutes early to fill out /update your paperwork. Or, you may print out your paperwork and complete it before your visit. You may also complete your paperwork online.
We also accept all major credit cards. these will be subject to a 2% transaction fee.
